AlloDerm is an acellular dermal matrix allograft that requires an overlying ultra-thin epithelial autograft. It has been known to minimize the disadvantages of thick split thickness autograft such as scar contractures, delayed healing and scarring of the donor site, and the risk of infection. Freeze dried AlloDerm is an acelluar, immunologically inert dermal matrix. It is free of epidermal cells, fibroblasts and endothelial cells which are the usual targets in immune response, while elastin proteoglycan and the basement membrane complex have been retained. We report 10 cases of reconstructionin in postburn scar contractures using AlloDerm with ultra-thin meshed autograft. Good cosmetic outcome was obtained with least contractures and a faster healing postoperatively. Further histological studies to document the vascularization and epithelization process should be sought.
